ABSTRACT

Introduction: Osteomas are benign tumors of the bone usually seen in craniofacial bones which arise from either the cortex or medulla. They are usually asymptomatic with slow painless growth. Multiple osteomas are often associated with Gardners syndrome. Solitary osteomas are typically non syndromic. Case Presentation: The patient presented with remarkable facial asymmetry following the growth in the mandible. Multiple os- teomas were noted which invoked suspicion of a syndrome. On endoscopic evaluation the patient was found to have multiple intestinal polyps, pathgnomonic for Gardners syndrome. Management and prognosis: The lesion was surgically removed under local anaesthesia. as an excisional biopsy and the asymmetry was rectified. Histopathologic examination revealed it to be ivory osteoma with dense compact bone and less mar- row spaces. The jaw lesions would precede the development of colonic polyps and therefore may contribute to early diagnosis of Gardner’s syndrome. There is also a chance of malignant evolution associated with the polyps Conclusion: People with the condition have a higher risk of developing other family adenomatous polyp (FAP)related cancers including pancreatic cancer and liver cancer. The patient should be in close follow up

Introducción: Un apoyo plantar incorrecto puede considerarse un factor etiológico de asimetrías faciales y cráneo-mandibulares pues modifica, de forma instantánea, la relación entre el maxilar y la mandíbula. Por tanto, resulta vital identificar la etiología de estas asimetrías para establecer diagnósticos y tratamientos certeros. Objetivo: Determinar la influencia del apoyo plantar en la postura corporal, la simetría facial y cráneo-mandibular en adolescentes. Métodos: Se realizó un estudio descriptivo y transversal de enero a septiembre de 2019 con 180 adolescentes que asistieron al servicio de Ortodoncia de la Clínica Docente de Especialidades «Victoria de Santa Clara», los cuales cumplían con criterios de selección de la investigación. Se analizaron variables faciales, posturales y cefalométricas. Se determinó: la simetría facial mediante el análisis de líneas faciales, el apoyo plantar al calcular el índice cavitario, la postura corporal, según la prueba de Di Rocca, y la simetría mandibular con el método modificado de Kurt y Uysal. Se siguieron las normas éticas y fueron aplicadas la prueba de Fisher, la de McNemar, y la técnica de conglomerado. Resultados: Predominaron los adolescentes con apoyo plantar asimétrico y los pies varo. Del total de casos con asimetría facial, 72,53 % presentó apoyo plantar asimétrico, y 39,44 % planos biilíaco y biclavicular desequilibrados y divergentes. La asimetría mandibular se observó en el 56,11%, la mayoría con apoyo plantar asimétrico. Conclusiones: Se pudo constatar la influencia del apoyo plantar en la postura corporal, la simetría facial y cráneo-mandibular, pues se observó un alto grado de relación entre las variables estudiadas.


Introduction: an incorrect plantar support can be considered an etiological factor of facial, mandibular and cranial asymmetries since it instantly modifies the relationship between the maxilla and mandible. Therefore, it is vital to identify the etiology of these asymmetries to establish accurate diagnoses and treatments. Objective: to determine the influence of plantar support on body posture and on facial, mandibular and cranial symmetries in adolescents. Methods: a descriptive and cross-sectional study was carried out from January to September 2019 with 180 adolescents who were seen in the Orthodontics service at "Victoria de Santa Clara" Specialty Teaching Dental Clinic, who met the research selection criteria. Facial, postural and cephalometric variables were analyzed. Facial symmetry was determined through the analysis of facial lines, plantar support by means of cavity index, body posture according to the Di Rocca test, and mandibular symmetry with the modified method of Kurt and Uysal. Ethical standards were followed and Fisher's and McNemar's tests as well as clustering technique were applied. Results: adolescents with asymmetric plantar support and varus feet predominated. The 72.53% had asymmetric plantar support from the total cases with facial asymmetry, and 39.44% had unbalanced and divergent biiliac and biclavicular planes. Mandibular asymmetry was observed in 56.11%, mostly with asymmetric plantar support. Conclusions: the influence of plantar support on body posture and on facial, mandibular and cranial symmetries was posible to verify since a high degree of relationship was observed among the variables studied.

ABSTRACT

A esclerodermia localizada (morfeia linear) é uma doença rara que causa distrofia e assimetria faciais em mulheres jovens. É relatado o caso de uma paciente de 20 anos que foi tratada com enxertos compostos de gordura. Após a coleta do lipoaspirado de gordura subcutânea profunda, a microgordura e a nanogordura não filtrada foram processadas separadamente para formar enxertos compostos de gordura. Estes foram meticulosamente transplantados em camadas, para reconstruir o subcutaneo da face da paciente. Após um ano, observou-se boa pega de gordura com correção bem-sucedida da assimetria e restabelecimento do equilíbrio facial.


Localized scleroderma (linear morphea) is a rare disease that causes facial dystrophy and asymmetry in young women. We report the case of a 20-year-old female patient who was treated with novel composite fat grafts. After extracting lipoaspirate from deep subcutaneous fat, the microfat and the unfiltered nanofat were processed separately, and amalgamated, obtaining composite fat grafts. These were meticulously transplanted layer by layer to rebuild the subcutaneous tissue of the patient's face. After one year, a good volume of fat retention was observed with successful correction of asymmetry and restoration of facial balance.

ABSTRACT

Purpose: To evaluate the frequency of facial asymmetry parameters in patients with head tilt versus those with head turn. Methods: This cross-sectional comparative study was performed on 155 cases, including 58 patients with congenital pure head turn due to Duane retraction syndrome (DRS), 33 patients with congenital pure head tilt due to upshoot in adduction or DRS, and 64 orthotropic subjects as the control group. The facial appearance was evaluated by computerized analysis of digital photographs of patients' faces. Relative facial size (the ratio of the distance between the external canthus and the corner of the lips of both face sides) and facial angle (the angular difference between a line that connects two external canthi and another line that connects the two corners of the lips) measured as quantitative facial parameters. Qualitative parameters were evaluated by the presence of one-sided face, cheek, and nostril compression; and columella deviation. Results: The facial asymmetry frequency in patients with head tilt, head turn, and orthotropic subjects was observed in 32 (97%), 50 (86.2%), and 22 (34.3%), respectively (P < 0.001). In patients with head tilt and head turn, the mean facial angle was 1.78º ± 1.01º and 1.19º ± 0.84º, respectively (P = 0.004) and the mean relative facial size was 1.027 ± 0.018 and 1.018 ± 0.014, respectively (P = 0.018). The frequencies of one-sided nostril compression, cheek compression, face compression, and columella deviation in patients with pure head tilt were found in 19 (58%), 21 (64%), 19 (58%), and 19 (58%) patients, respectively, and in patients with pure head turn the frequencies were observed in 42 (72%), 37 (63%), 27 (47%), and 43 (74%), respectively. All quantitative and qualitative facial asymmetry parameters and facial asymmetry frequencies were significantly higher in head tilt and head turn patients as compared to the control group (P < 0.001). Conclusion: All facial asymmetry parameters in patients with head tilt and head turn were significantly higher than orthotropic subjects. The quantitative parameters such as relative facial size and facial angle were significantly higher in patients with pure head tilt than pure head turn. The results revealed that pure head tilt was associated with a higher prevalence of facial asymmetry than pure head turn.

@#Facial symmetry evaluation has always been a hot topic of concern for doctors who engage in the study of facial beauty disciplines such as orthodontics, dentistry, and plastic surgery. Although scholars at home and abroad have carried out much research on the evaluation of facial symmetry with a variety of emerging technologies and methods, there is still a lack of unified standards for the evaluation of facial asymmetry due to the complexity of the content and methods and individual subjectivity. Facial asymmetry involves changes in the length, width and height of the face. It is a complex dental and maxillofacial malformation whose early identification and accurate evaluation are particularly important. Clinically, in addition to the necessary dental and maxillofacial examinations, it is also necessary to evaluate facial asymmetry with the help of corresponding auxiliary methods. This paper gives a summary of the commonly used three-dimensional evaluation methods. The evaluation methods of facial asymmetry can be divided into 5 categories: qualitative analysis, quantitative analysis, dynamic analysis, mathematical analysis, and artificial intelligence analysis. After the analysis and summarization of the characteristics, advantages and limitations of each method in clinical applications, it is found that although these methods vary in accuracy, evaluation scope, diagnosis nature and calculation method, etc., the three-dimensional evaluation methods are more objective, more accurate and more convenient and will become the mainstream evaluation method for facial asymmetry with further development of three-dimensional measurement technologies.

Introducción: la asimetría facial es una condición notoria en el tercio inferior de la facie del sujeto y entre los tercios medio y superior, en este último será menos evidente esta condición, de tal manera que podría considerarse como una de las molestias de mayor incidencia en pacientes con necesidades o no de terapia or- todóncica. Objetivo: evaluar mediante una revisión de la literatura los tratamientos ortodóncico-quirúrgicos de pacientes con asimetría facial. Material y métodos: la literatura se seleccionó mediante una búsqueda en las bases de datos electrónicas: PubMed, Scopus, Web of Science. Las palabras clave utilizadas fueron: facial asymmetry, asymmetry, surgical treatment, surgical orthodontic treatment. La búsqueda se restringió a artículos en inglés publicados del año 2011 al 2021. Resultados: después de aplicar los criterios de inclusión y exclusión en total se obtuvieron y revisaron 27 artículos. Se realizó la revisión de literatura del tratamiento ortodóncico-quirúrgico en pacientes con asimetría facial. Conclusión: es preciso el abordaje ortodóntico quirúrgico para la corrección de la asimetría facial, del protocolo dependerá la situación clínica y la elección de tratamiento ortodóntico quirúrgico, lo que brindará mejoras significativas en la simetría facial (AU)


Introduction: facial asymmetry, is a notorious condition in the lower third of the subject's facie and between the middle and upper thirds, in the latter this condition will be less evident; in such a way, it could be considered as one of the discomforts of greater incidence in patients with needs or not of orthodontic therapy. Objective: to evaluate by means of a literature review the orthodontic-surgical treatment of patients with facial asymmetry. Material and methods: the literature was selected through a search in the following electronic databases: PubMed, Scopus, Web of Science. The keywords used were: facial asymmetry, asymmetry, surgical treatment, surgical orthodontic treatment. The search was restricted to articles in English published from 2011 to 2021. Results: after applying the inclusion and exclusion criteria, a total of 27 articles were obtained and reviewed. The literature review of orthodontic-surgical treatment in patients with facial asymmetry was performed. Conclusion: surgical orthodontic approach is necessary for the correction of facial asymmetry, the protocol will depend on the clinical situation, the choice of surgical orthodontic treatment, which will give significant improvements in facial symmetry (AU)

Introdução: A paralisia facial necessita de um tratamento multidisciplinar e as opções cirúrgicas são individualizadas para cada paciente e conforme a experiência da equipe médica. O Retalho Ortodrômico Temporal (ROT) é uma forma de correção bem documentada na literatura, com resultados satisfatórios. Um dos passos de sua realização é a coleta da fáscia lata para ponte entre o tendão temporal e os lábios. O objetivo é propor uma padronização da quantidade necessária de fáscia lata e técnica de coleta simplificada. Métodos: Descrição cirúrgica da medida de fáscia necessária para o procedimento e sua coleta. Resultados: Procedimento replicável e seguro conforme experiência do autor sênior. Conclusão: A tática de coleta proposta pode facilitar a realização deste procedimento e torná-lo mais seguro para os pacientes em diferentes serviços de saúde.


Introduction: Facial paralysis requires a multidisciplinary treatment, and surgical options are individualized for each patient according to the medical team's experience. The Temporal Orthodromic Flap (TOF) is a well-documented form of correction in the literature, with satisfactory results. One of the steps of its accomplishment is the collection of the fascia lata to bridge between the temporal tendon and the lips. The objective is to propose a standardization of the required amount of fascia lata and a simplified collection technique. Methods: Surgical description of the fascia measure necessary for the procedure and collection. Results: Replicable and safe procedure according to the senior author's experience. Conclusion: The proposed collection tactic can facilitate the performance of this procedure and make it safer for patients in different health services.

Introducción: Estudios previos han demostrado diferencias en la percepción de asimetrías dentales y faciales entre profesionales de la odontología y personas no expertas. Sin embargo, la literatura sobre las diferencias en la percepción de las asimetrías del mentón es limitada. Objetivo: Comparar la percepción estética de las asimetrías del mentón entre ortodoncistas, odontólogos especialistas, estudiantes y pacientes de la Facultad de Odontología de la Universidad Central del Ecuador. Métodos: Estudio observacional, analítico y transversal. El universo estuvo conformado por 36 ortodoncistas, 77 odontólogos especialistas, 96 estudiantes y 2580 pacientes de esta Facultad. El tamaño de la muestra fue de 30 participantes para cada grupo. Se modificó digitalmente la posición transversal del mentón de 0° a 6° en las fotografías de dos sujetos (hombre-mujer) ecuatorianos. La evaluación se realizó por medio de una escala visual análoga. Resultados: Las fotografías de asimetrías del mentón con 0° de desviación fueron calificadas como muy estéticas y las asimetrías con 6° fueron poco estéticas para los cuatro grupos. Los ortodoncistas, odontólogos especialistas y estudiantes consideraron como límite estético los 2° de desviación del mentón para la fotografía del sujeto femenino y masculino. Los límites estéticos fueron los 2° de desviación para el sujeto femenino y 3° para el sujeto masculino. Se encontraron diferencias significativas entre los grupos de evaluadores y las fotografías (p < 0,05). Conclusiones: Los ortodoncistas percibieron la estética facial cercana a la norma de la asimetría del mentón, que tiene un papel importante en la percepción de la estética facial(AU)


Introduction: Previous studies have shown differences in the perception of dental and facial asymmetries between dental professionals and non-experts. However, the literature on differences in the perception of chin asymmetries is limited. Objective: Compare the aesthetic perception of chin asymmetries among orthodontists, specialist dentists, students and patients of the Faculty of Dentistry of the Central University of Ecuador. Methods: Observational, analytical and cross-sectional study. The universe was made up of 36 orthodontists, 77 specialist dentists, 96 students and 2580 patients from this Faculty. The sample size was 30 participants for each group. The transverse position of the chin was digitally modified from 0° to 6° in the photographs of two Ecuadorian subjects (male-female). The evaluation was carried out by means of an analogous visual scale. Results: The photographs of asymmetries of the chin with 0° of deviation were qualified as very aesthetic and the asymmetries with 6° were unsightly for the four groups. Orthodontists, specialist dentists and students considered as an aesthetic limit the 2nd deviation of the chin for the photography of the female and male subject. The aesthetic limits were 2° of deviation for the female subject and 3° for the male subject. Significant differences were found between the groups of evaluators and the photographs (p < 0.05). Conclusions: Orthodontists perceived facial aesthetics close to the norm of chin asymmetry, which has an important role in the perception of facial aesthetics(AU)

ABSTRACT

Hemifacial hyperplasia (HH) is a rare congenital condition involving enlargement of one or more tissues of the face. The treatment is surgically challenging and requires expertise. This manuscript aims to report two similar appearing HH but warranting different surgical treatment. A 19?year?old female and a 14?year?old boy presented with right facial asymmetry since birth and sought correction of the same. Surgical treatment was planned. Based on clinical history, diagnosis and imaging, HH was diagnosed. The first case was entirely a soft tissue abnormality that was treated with debulking while the second case had involvement of facial bones, necessitating surgical recontouring. The facial asymmetry was addressed. Healing was uneventful. Though the aesthetical concern and appearance of the two cases of HH were same, the treatment vastly differed. This was based on the source of asymmetry. Proper diagnosis and informed decision are a key for successful surgical outcome.

Introdução: A paralisia facial é um quadro que pode gerar algumas sequelas, e às vezes apenas as estéticas podem ser melhoradas. Com base nisso, objetivou-se relatar um caso em que foi utilizada uma tela de polipropileno e poliglecaprone com finalidade de elevar as estruturas da hemiface direita. Relato de Caso: A paciente apresentava paralisia em hemiface direita, com selamento palpebral incompetente, desvio de comissura labial e sem movimento da musculatura temporal. Foi realizada uma incisão abaixo da costeleta, pré e retroauricular, com descolamento do retalho cutâneo em toda a hemiface direita. Após levantar o sistema musculoaponeurótico superficial e fixá-lo com fios de mononylon, foi colocada a tela de polipropileno e poliglecaprone na região do terço médio e fixada com monocryl. Foi realizada cantotomia e cantopexia lateral da pálpebra direita. No pós-operatório imediato a paciente evoluiu sem edemas, retrações ou abaulamentos, e após um ano e sete meses apresenta total integração da tela, sem retração, fibrose ou recidiva. Discussão: A escolha do tratamento estético de paralisia facial depende da causa e duração da lesão, mas existem diversas formas de fazê-lo. Entre as ideias mais novas, estão o uso de células tronco e materiais aloplásticos e, seguindo essa segunda linha, a tela de polipropileno e poliglecaprone pode ser pensada como uma técnica viável, como foi neste caso relatado.


Introduction: Facial paralysis is a condition that can cause some sequelae, and sometimes only aesthetics can be improved. Based on this, the objective was to report a case in which a polypropylene and poliglecaprone mesh was used in order to raise the structures of the right hemiface. Case Report: The patient had paralysis in the right hemiface, with incompetent eyelid sealing, deviation of the labial commissure and no movement of the temporal musculature. An incision was made below the cutlet, pre and retroauricular, with detachment of the skin flap across the right hemiface. After lifting the superficial musculoaponeurotic system and fixing it with mononylon threads, the polypropylene and poliglecaprone mesh was placed in the middle third region and fixed with monocryl. Canthotomy and lateral canthopexy of the right eyelid were performed. In the immediate postoperative period, the patient evolved without edema, retraction or bulging, and after one year and seven months, she had complete integration of the mesh, with no retraction, fibrosis or recurrence. Discussion: The choice of aesthetic treatment for facial paralysis depends on the cause and duration of the injury, but there are several ways to do it. Among the newer ideas are the use of stem cells and alloplastic materials, and following this second line, the polypropylene and poliglecaprone canvas can be thought of as a viable technique, as was reported in this case.

Purpose: To assess the association of congenital severe ptosis with facial asymmetry compared with general population with same age group with no lid abnormality. Assessment of all 30 patient Method: s with severe unilateral congenital ptosis compared with age matched patients attending OPD with no lid abnormality was done. Various parameters were used to measure facial anthropometry. We found significant asymmetry of upper and lower face index in 4 patients Result: . Upper and lower face index was smaller on ptotic side as compared to the normal side. The observation suggests that severe congenital ptosi Conclusion: s could be associated with facial asymmetry and a trend towards smaller face on the affected side in a certain number of cases.

RESUMEN Se presentó un paciente masculino de 8 años de edad, de la raza negra, a la consulta del servicio de Cirugía Maxilofacial del Hospital Américo Boavida de Luanda, en la República de Angola, en el mes de marzo de 2018. Los familiares refirieron una gran deformidad facial de crecimiento lento y progresivo, que se acompañaba de dificultad para respirar por las fosas nasales. Se le realizó un diagnóstico presuntivo de displasia fibrosa facial monostótica del maxilar derecho. Fue intervenido quirúrgicamente bajo anestesia endotraqueal, previa realización de las pruebas para la valoración de la apertura de la boca, donde se tomaron los incisivos superiores e inferiores como referencia: obtuvo una clasificación en clase II (2,6-3 cm) y la escala de Mallampati en grado IV. Se logró superar las dificultades para la intubación. Se le realizó exéresis tridimensional con abordaje de Weber-Ferguson y reconstrucción estética facial.


ABSTRACT An eight-years-old black male patient was attended in the Maxillofacial Surgery Service consultation at the Americo Boavida Hospital in Luanda, Angola, in march 2018. Relatives reported a huge facial deformity with characteristic variable (slow growth) and evolve with time. Also they referred presence of difficulty in breathing through the nostrils, associated with this anomaly. A presumptive diagnosis of monostotic facial fibrous dysplasia of the right maxillary segment was made. He underwent surgery under endotracheal anesthesia, after undergoing tests for the evaluation of the range of mouth opening, where the upper and lower incisors were taken as references: classification obtained in test was Class II (2.6-3 cm) and the Mallampati scale was Grade IV. The intubation procedures difficulties were overcome. Three-dimensional excision with Weber-Ferguson approach and reconstructive plastic surgery was performed.


RESUMO Paciente do sexo masculino, 8 anos, de raça negra, apresentou-se ao serviço de Cirurgia Buco Maxilo Facial do Hospital Américo Boavida, em Luanda, na República de Angola, em março de 2018. Os familiares relataram grande deformidade facial lenta e progressiva, que foi acompanhada de dificuldade em respirar pelas narinas. Foi feito diagnóstico presuntivo de displasia fibrosa facial monostótica da maxila direita. Foi operado sob anestesia endotraqueal, após a realização dos testes de avaliação da abertura da boca, onde foram tomados como referência os incisivos superiores e inferiores: obteve a classificação de classe II (2,6-3 cm) e a escala de Mallampati no grau IV. As dificuldades na intubação foram superadas. A exérese tridimensional foi realizada com a abordagem de Weber-Ferguson e cirurgia plástica reconstrutiva.

ABSTRACT

ABSTRACT@#Granulomatosis with polyangiitis (GPA) is a rare multisystem disease. Although GPA is rare, it commonly presents in a localised stage where its manifestation involves the upper or lower respiratory tract before progressing to a generalised stage. Therefore, most patients with GPA will visit an oral surgeon or an otolaryngologist to seek treatment. However, the diagnosis of GPA is often delayed as GPA is not frequently considered as a differential diagnosis in common oral and facial diseases. The lack of gold standard investigation for the diagnosis of GPA makes management of this case, a diagnostic conundrum. We herein report a patient who was diagnosed with bilateral acute otitis media and left mastoiditis complicated with facial nerve palsy, and later developed tongue ulceration one month after his initial presentation. The ear, facial and oral symptoms represent a diagnostic red herring to a full-blown generalised stage of GPA.

ABSTRACT Objective: To describe an infant with craniofacial microsomia and recurrent respiratory distress associated with aberrant right subclavian artery in order to review its most frequent congenital anomalies and alert the pediatrician to its rarer and more severe complications. Case description: This case report involves an 18-month-old male infant, only son of non-consanguineous parents. At birth, the child presented craniofacial dysmorphisms (facial asymmetry, maxillary and mandibular hypoplasia, macrostomia, grade 3 microtia, and accessory preauricular tag) restricted to the right side of the face. Additional tests showed asymmetric hypoplasia of facial structures and thoracic hemivertebrae. No cytogenetic or cytogenomic abnormalities were identified. The patient progressed to several episodes of respiratory distress, stridor, and nausea, even after undergoing gastrostomy and tracheostomy in the neonatal period. Investigation guided by respiratory symptoms identified compression of the esophagus and trachea by an aberrant right subclavian artery. After surgical correction of this anomaly, the infant has not presented respiratory symptoms and remains under multidisciplinary follow-up, seeking rehabilitation. Comments: Craniofacial microsomia presents a wide phenotypic variability compared to both craniofacial and extracraniofacial malformations. The latter, similarly to the aberrant right subclavian artery, is rarer and associated with morbidity and mortality. The main contribution of this case report was the identification of a rare anomaly, integrating a set of malformations of a relatively common condition, responsible for a very frequent complaint in pediatric care.


RESUMO Objetivo: Descrever lactente com microssomia craniofacial e desconforto respiratório recorrente associado à artéria subclávia direita aberrante, com o intuito de revisar as anomalias congênitas mais frequentes e alertar o pediatra sobre suas complicações mais raras e de maior gravidade. Descrição do caso: Lactente do sexo masculino, 18 meses de idade e filho único de casal não consanguíneo. Ao nascimento, foram observadas dismorfias craniofaciais (assimetria facial, hipoplasia maxilar e mandibular, macrostomia, microtia grau 3 e apêndice na linha trago-oral) restritas ao lado direito da face. Os exames complementares evidenciaram hipoplasia assimétrica da face e hemivértebras torácicas. Não foram identificadas anormalidades citogenéticas ou citogenômicas. O paciente evoluiu com diversos episódios de desconforto respiratório, estridor e náuseas, mesmo tendo realizado gastrostomia e traqueostomia no período neonatal. A investigação direcionada para os sintomas respiratórios identificou a compressão de esôfago e traqueia por uma artéria subclávia direita aberrante. Após a correção cirúrgica dessa anomalia, o lactente não tem apresentado sintomas respiratórios e mantém seguimento multidisciplinar buscando reabilitação. Comentários: A microssomia craniofacial apresenta grande heterogeneidade fenotípica em relação às malformações tanto craniofaciais como extracraniofaciais. Estas últimas, como a artéria subclávia direita aberrante, são mais raras e associadas à morbimortalidade. A principal contribuição deste relato de caso foi o reconhecimento de uma anomalia rara, integrando um conjunto de malformações de uma condição relativamente comum e sendo responsável por uma queixa muito frequente no atendimento pediátrico.

OBJECTIVE@#To automatically construct lips symmetry reference plane (SRP) based on posed smile, and to evaluate its advantages over conventional digital aesthetic design.@*METHODS@#Eighteen subjects' three-dimensional facial and dentition data were gathered in this study. The lips SRP of experimental groups were used with the standard weighted Procrustes analysis (WPA) algorithm and iterative closest point (ICP), respectively. A reference plane defined by experts based on regional ICP algorithm, served as the truth plane. The angle error values between the lips SRP of WPA algorithm in the experimental groups and the truth plane were evaluated in this study, and the lips SRP of ICP algorithm of the experimental groups was calculated in the same way. The lips SRP based on posed smile as a reference for aesthetic design and evaluate preliminary clinical application.@*RESULTS@#The average angle error between the lips SRP of WPA algorithm and the truth plane was 1.78°±1.24°, which was smaller than that between the lips SRP of ICP and the truth plane 7.41°±4.31°. There were significant differences in the angle errors among the groups (P < 0.05). In the aesthetic design of anterior teeth, automatically constructing the lips SRP of WPA algorithm based on posed smile and the original symmetry plane by re-ference compared with the prosthetic design, the subjects' scores on the lips SRP of WPA algorithm based on posed smile (8.48±0.57) were higher than those on the original symmetry plane (5.20±1.31).@*CONCLUSION@#Automatically constructing the lips SRP of WPA algorithm based on posed smile was more accurate than ICP algorithm, which was consistent with the truth plane. Moreover, it can provide an important reference for oral aesthetic diagnosis and aesthetic analysis of the restoration effect. In the aesthetic design of anterior teeth, automatically constructing the lips SRP of WPA algorithm based on posed smile can improve the patients' satisfaction in esthetic rehabilitation.

ABSTRACT: Osteochondroma is a common benign bone tumor and although its involvement in facial bones is rare (0.6 %), it shows a strong predilection for developing in the mandibular condyle, causing occlusal changes and facial asymmetry. The aim of this study was to report a clinical case and the treatment of a 43-year-old male patient, who was diagnosed with condylar osteochondroma. Clinically, the patient presented an increase in volume in the right condylar region, deviation of the mandible to the left side and malocclusion. His main complaint was his facial asymmetry. Imaging exams showed changes in the size and morphology of the right condyle, and signs of hyper-capture on scintigraphy. The hypothesis of condylar hyperplasia and osteochondroma was raised. A virtual planning and execution of an orthognathic surgery of the jaws was performed to correct the occlusion and facial asymmetry, besides a low condylectomy to remove the lesion. Procedures such as high, low, proportional condylectomy, orthognathic surgery and TMJ reconstructive surgery are alternatives for the treatment of these pathologies. In our case, the orthognathic surgery combined with a condylectomy proved to be promising, to correct the occlusion and the facial asymmetry of the patient.


RESUMEN: El osteocondroma es un tumor óseo benigno común y aunque su afectación en los huesos faciales es rara (0,6 %), muestra una fuerte predilección por desarrollarse en el cóndilo mandibular, causando cambios oclusales y asimetría facial. El objetivo de este trabajo fue informar de un caso clínico y el tratamiento de un paciente de sexo masculino de 43 años, que fue diagnosticado con osteocondroma cóndilar. Clínicamente, el paciente presentaba un aumento en el volumen en la región condilar derecha, desviación de la mandíbula hacia el lado izquierdo y maloclusión. Su principal molestia era su asimetría facial. Los exámenes por imágenes mostraron cambios en el tamaño y la morfología del cóndilo derecho, y signos de hiper-captura en la centellografía. Se planteó la hipótesis de hiperplasia condilar y osteocondroma. Se realizó una planificación virtual y ejecución de una cirugía ortognática de la mandíbula para corregir la oclusión y la asimetría facial, además de una baja condilectomía para extirpar la lesión. Procedimientos como la condilectomía alta, baja y proporcional, la cirugía ortognática y la cirugía reconstructiva de la ATM son alternativas para el tratamiento de estas patologías. En nuestro caso, la combinación de la cirugía ortognática y una condiloectomía resultó ser prometedora, para corregir la oclusión y la asimetría facial del paciente.

SUMMARY: Unilateral condylar hyperplasia (UCH) is an alteration of the mandibular condyle growth. The aim of this study was to evaluate condyle volume, surface area, and Morphological Index (MI) differences between the affected condyle and an unaffected one in patients with UCH, evaluated through 3D reconstructions cone beam computed tomography (CBCT) images by two open-source softwares. A retrospective cross-sectional study of 16 patients with a certain UCH, 9 females and 7 males with mean age 25.13 ± 6.8 years was made. The image obtained from the CBCT of each condyle were reconstructed using the open-source software 3D SLICER 4.6 ®. The volumetric and area measurements of the 3D reconstruction of the mandibular condyle were made using the open-source soft- ware NETFABB basic 5.0 ®. The mean condylar volume of the hyperplastic condyles was 2.07 ± 1.51 cm3 and the non-hyperplastic condyles was 1.16 ± 0.82 cm3 (p<0.05). The mean area surface of the hyperplastic condyle was 11.77 ± 3.71 cm2 and the non-hyperplasic condyle mean was 8.05 ± 2.17 cm2 (p < 0.05). The mean area surface difference was 3.72 ± 3.57 cm2 (28.0 %). The MI of the hyperplastic condyle was 1.8 ± 0.3 mm and the non-affected condyle was 1.3 ± 0.6 mm (p < 0.05). The use of open-source software for 3D reconstruction with manual segmentation for evaluation of the volume and the condylar surface is a valid tool available to the clinic in the diagnosis and monitoring of patients with condylar hyperplasia.


RESUMEN: La hiperplasia condilar unilateral (HU) es una alteración del crecimiento del cóndilo mandibular. El objetivo de este estudio fue evaluar en pacientes el volumen del cóndilo, el área de superficie y las diferencias del índice morfológico (IM) entre el cóndilo afectado y el no afectado en pacientes, mediante tomografía computarizada de haz cónico (TCHC), por medio de dos softwares. Se realizó un estudio transversal retrospectivo de 16 pacientes con determinada HU, 9 mujeres y 7 hombres con edad media 25,13 ± 6,8 años. La imagen obtenida del TCHC de cada cóndilo se reconstruyó utilizando el software de código abierto 3D SLICER 4.6 ®. Las medidas volumétricas y de área de la reconstrucción 3D del cóndilo mandibular se realizaron utilizando el software de código abierto NETFABB basic 5.0 ®. El volumen condilar medio de los cóndilos hiperplásicos fue de 2,07 ± 1,51 cm3 y el de los cóndilos no hiperplásicos fue de 1,16 ± 0,82 cm3 (p <0,05). La superficie media del cóndilo hiperplásico fue de 11,77 ± 3,71 cm2 y la media del cóndilo no hiperplásico fue 8,05 ± 2,17 cm2 (p <0,05). La diferencia de superficie de área media fue 3,72 ± 3,57 cm2 (28,0 %). El IM del cóndilo hiperplásico fue de 1,8 ± 0,3 mm y el cóndilo no afectado fue de 1,3 ± 0,6 mm (p <0,05). Para el clínico, el uso de software de código abierto en la reconstrucción 3D con segmentación manual, para la evaluación del volumen y la superficie condilar, es una herramienta efectiva en el diagnóstico y tratamiento de pacientes con hiperplasia condilar.

RESUMEN Introducción: el cuerpo humano se mantiene en bipedestación gracias al equilibrio existente entre distintos planos quinesiológicos, la alteración de uno de ellos supondrá un desajuste de todo el conjunto. La mordida cruzada es una maloclusión frecuente en la práctica diaria, la cual precisa de un diagnóstico y plan de tratamiento precoz, pues las modificaciones que provoca a diferentes niveles se incrementan con la edad, lo que produce importantes asimetrías que pueden expresarse facial y corporalmente. Objetivo: determinar la convergencia ocular, la simetría facial y corporal en los niños de 5 a 12 años de edad afectados por mordidas cruzadas de las escuelas primarias de la zona urbana del municipio de Quemado de Güines. Métodos: se realizó un estudio descriptivo de corte transversal en las escuelas primarias de la zona urbana del municipio de Quemado de Güines en el período comprendido desde noviembre de 2014 hasta junio de 2016. Se seleccionó una muestra no probabilística que incluyó pacientes de 5 a 12 años con diagnóstico de mordida cruzada, los cuales no habían recibido tratamiento ortodóncico previo y fueron autorizados por sus padres o tutores para participar en el estudio. Resultados: predominaron los niños de 8 años de edad (40%). Solo 3 niños no mostraron trastorno de la convergencia ocular. Por otra parte, 5 niños fueron simétricos (simetría facial), y en relación a la simetría corporal: solamente 2 niños eran simétricos Conclusiones: se determinó que existe una significativa relación entre las mordidas cruzadas, la convergencia ocular y la simetría facial y corporal.


ABSTRACT Introduction: the human body remains standing thanks to the existing balance between different kinesiological planes; the alteration of one of them will suppose a mismatch in all body's structures. Crossbite is a frequent malocclusion in daily practice, which requires an early diagnosis and treatment plan, since the modifications it causes at different levels increase with age, which produces important asymmetries that can be expressed facially and bodily. Objective: to determine ocular convergence, facial and body symmetry in children affected by crossbites, aged 5 to 12 years and belonging to primary schools from the urban area of Quemado de Güines municipality. Methods: a cross-sectional descriptive study was carried out in primary schools from the urban area of Quemado de Güines municipality from November 2014 to June 2016. A non-probabilistic sample that included patients aged 5 to 12 years, diagnosed with crossbite, who were authorized to participate in the study by their parents or guardians and had not received previous orthodontic treatment was selected. Results: 8-year-old children predominated (40%). Only 3 children showed no ocular convergence disorder. On the other hand, 5 children were symmetric (facial symmetry), and in relation to body symmetry: only 2 children were symmetric. Conclusions: a significant relationship among crossbites, ocular convergence, facial and body symmetry was determined.

Abstract Objective: To analyze differences in vertical mandibular and trunk symmetry in orthodontic patients. Material and Methods: This was a cross-sectional study of 129 growing orthodontic patients who sought orthodontic treatment at the Dental Hospital Universitas Sumatera Utara, Indonesia. Mandibular symmetry index was observed with pre-treatment panoramic radiography based on Kjellberg's technique and trunk symmetry was evaluated based on questionnaires and visual observation. Vertical mandibular asymmetry was decided if the index of asymmetry was lower than 93.7%. The bivariate analysis used the chi-squared and Fisher's exact tests, with a significance level of 5%. Results: There was a significant association between vertical mandibular and trunk symmetry (p<0.05). The prevalence odds ratio for the association with vertical mandibular asymmetry was 3.007 (95% CI = 1.016-8.905) for trunk asymmetry. Conclusion: The necessity to consider trunk symmetry could be included in orthodontics treatment of any malocclusion with vertical mandibular asymmetry that might require a multidisciplinary approach in the future.

ABSTRACT Introduction: Skeletal posterior crossbite (SPCB) has a multifactorial etiology, as it may be caused by parafunctional habits, atypical position of the tongue, tooth losses and maxillary or mandibular transverse skeletal asymmetries. Skeletal involvement may lead to facial changes and an unfavorable aesthetic appearance. The treatment of SPCB diagnosed in an adult patient should be correctly approached after the identification of its etiologic factor. Surgically-assisted rapid maxillary expansion (SARME), one of the techniques used to correct SPCB in skeletally mature individuals, is an efficient and stable procedure for the correction of transverse discrepancies that may be performed in the office or in a hospital. Objective: This study discusses the results of asymmetrical SARME used to correct unilateral SPCB associated with transverse mandibular asymmetry. Conclusion: The treatment alternative used in the reported case was quite effective. At the end of the treatment, the patient presented adequate occlusion and facial aesthetics.


RESUMO Introdução: A mordida cruzada posterior esquelética (MCPE) apresenta etiologia multifatorial, podendo ser causada por hábitos parafuncionais, posição atípica da língua, perdas dentárias e assimetrias esqueléticas transversais da maxila ou da mandíbula. Alterações faciais podem estar presentes quando há envolvimento esquelético, levando a estética desfavorável. O tratamento da MCPE, quando diagnosticada no paciente adulto, requer abordagem correta, com identificação do fator etiológico. Entre as técnicas utilizadas para correção da MCPE em pacientes esqueleticamente maduros, cita-se, em especial, a Expansão Rápida de Maxila Assistida Cirurgicamente (ERMAC). Essa modalidade tem se mostrado bastante eficiente na correção dos problemas transversais, apresenta estabilidade e pode ser realizada em ambiente ambulatorial ou hospitalar. Objetivo: O objetivo do presente trabalho será discutir os resultados da ERMAC assimétrica para correção da MCPE unilateral associada a assimetria transversal da mandíbula. Conclusão: A alternativa de tratamento utilizada no caso relatado mostrou-se bastante eficiente. Ao fim do tratamento, o paciente apresentou adequada oclusão e boa estética facial.
